Could you please complete the below in confidence, we are gathering information following a complaint made against Cedar Hope Care Services to our regulator CQC. We would like to gather your views so that we can respond fully to the issues. You are welcome to respond to the specific items on this form or provide any further information not requested.

This information is strictly confidential and for HR use for purposes of informing the response to CQC you can be as detailed and specific as possible.

Do you think there are any inappropriate working relationships within the company?

Do you believe there are any issues around neglect and/or abuse of service users?

Any issues such as Staff sharing alcohol and smoking with the service users.

Do you have any issues regarding accessibility or availability of Manager, Deputy manager and Seniors for staff?

Do you feel supported, protected and feel you have a safe space to discuss issues with management and/or supervisors?

Do you have any concerns about Medication errors that have taken place and not been reported?

Do you feel that there is equal and fair treatment of all staff by Manager, Deputy and Seniors?

Are you aware of any appointments that have been missed by clients?

Do you feel that there are any issues of staff not getting their contracted hours?

Are you aware of clients being forced to go on any trips when they do not want to?

Are you aware of any issues in relation to professional boundaries e.g. how managers and staff relate to each other and how staff members relate to service users?
